When this type of pain is suspected, a precise diagnostic tool is needed to confirm it. This is where a Medial Branch Block (MBB) comes in.

2. Understanding Facet Joint Pain: The "Hidden" Cause
Your spine is made up of a series of bones (vertebrae) stacked on top of each other. At the back of each vertebra are small, paired joints called facet joints (or zygapophyseal joints). These joints are lined with cartilage and allow your spine to bend and twist smoothly.
Over time, or due to injury, the cartilage in these joints can wear down, leading to facet joint arthritis (spondylosis). This condition causes pain, stiffness, and inflammation. The challenge is that facet joint pain often mimics other spinal conditions, making it difficult to diagnose with imaging like MRI alone.
3. What is a Medial Branch Block? The Science Explained
A Medial Branch Block is a diagnostic injection procedure. Its primary goal is not to provide long-term treatment, but to confirm whether the facet joints are the source of your pain.
Each facet joint is served by tiny nerves called medial branch nerves. These nerves transmit pain signals from the facet joint to the brain. A medial branch block involves injecting a small amount of local anesthetic (numbing medicine) directly onto these specific nerves.
If your pain is coming from the facet joint, the anesthetic will temporarily block the pain signals, and you should experience significant pain relief during the period the medication is active.
4. What to Expect: The MBB Procedure at APSI Wellness
The procedure is performed in our state-of-the-art facility and is designed to be as comfortable as possible.
Before the Procedure
- You will have a consultation to review your history and ensure you are a good candidate.
- Certain medications may need to be adjusted.
During the Procedure
- You will lie face down on an exam table.
- The skin over the injection site will be cleaned and numbed.
- Using fluoroscopic (X-ray) guidance, the physician will guide a small needle to the medial branch nerves.
- A small amount of local anesthetic is injected.
- The procedure typically takes only 15-20 minutes.
After the Procedure
- You will be asked to monitor your pain levels closely.
- A "pain diary" is often used to track your relief.
- The anesthetic typically wears off within a few hours.
5. Interpreting Your Results: The Diagnostic Power of MBBs
The success of a medial branch block is determined by how much pain relief you experience:
- Positive Response (≥80% relief): This strongly suggests that the facet joints are a significant source of your pain. You may then be a candidate for a longer-lasting procedure called Radiofrequency Ablation (RFA).
- Negative or Partial Response (<50% relief): This indicates that the facet joints are likely not the primary cause of your pain, and other causes should be explored.
